Box 2042, Nakuru in the Republic of Kenya, formerly known as Silvia Jepchumba Tanui, formally and absolutely renounced and abandoned the use of her former name Silvia Jepchumba Tanui, and in lieu thereof assumed and adopted the name Judy Jepchumba Tanui, for all purposes and authorize and request all persons at all times to designate, describe and address her by her assumed name Judy Jepchumba Tanui only.
