Box 957–00621, Nairobi in the Republic of Kenya, the said infant, formerly known as John Irvin Abyong Odongo, formally and absolutely renounced and abandoned the use of his former name John Irvin Abyong Odongo, and in lieu thereof assumed and adopted the name John Irvin Oganga, for all purposes and authorize and request all persons at all times to designate, describe and address her by her assumed name Charity Thuguri Miles only.
