Box 90222–80100, Mombasa in the Republic of Kenya, formerly known as Safari Joseph Ngala, formally and absolutely renounced and abandoned the use of my former name Safari Joseph Ngala, and in lieu thereof assumed and adopted the name Yusuf Gandi Ngala, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Yusuf Gandi Ngala only.
