Box 714–80100, Mombasa in the Republic of Kenya, formerly known as Joseph Ngala Mwandonga, formally and absolutely renounced and abandoned the use of my former name Joseph Ngala Mwandonga, and in lieu thereof assumed and adopted the name Joseph Kombe Ngala, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Joseph Kombe Ngala only.
