Box 44706– 00100, Nairobi in the Republic of Kenya, formerly known Kanta Devji Karsan, formally and absolutely renounced and abandoned the use of my former name Kanta Devji Karsan, and in lieu thereof assumed and adopted the name Kanta Shivji Lalji Gajiparia, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Kanta Shivji Lalji Gajiparia only.
