Box 51723–00100, Nairobi in the Republic of Kenya, formerly known as Grace Njambi Karanja alias Grace Karanja Mwangi, formally and absolutely renounced and abandoned the use of my former name Grace Njambi Karanja alias Grace Karanja Mwangi, and in lieu thereof assumed and adopted the name Grace Njambi Kagwe, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Grace Njambi Kagwe only.
