Box 120–80302, Taveta in the Republic of Kenya, formerly known as Faith Mbinya Muia, formally and absolutely renounced and abandoned the use of my former name Faith Mbinya Muia, and in lieu thereof assumed and adopted the name Jane Kavinya Muiya, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Jane Kavinya Muiya only.
