Box 9284– 00100, Nairobi in the Republic of Kenya, formerly known Jeremy Waikwa, formally and absolutely renounced and abandoned the use of his former name Jeremy Waikwa, and in lieu thereof assumed and adopted the name Jeremy Waikwa Omondi, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Jeremy Waikwa Omondi only.
