Box 75720–00200, Nairobi in the Republic of Kenya, formerly known Henry Njoroge Kamau, formally and absolutely renounced and abandoned the use of my former name Henry Njoroge Kamau, and in lieu thereof assumed and adopted the name Henry Mburu Kamau, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Henry Mburu Kamau only.
