Box 55962– 00200, Nairobi in the Republic of Kenya, formerly known as Laureen Noelle Awino Otwala, formally and absolutely renounced and abandoned the use of her former name Laureen Noelle Awino Otwala, and in lieu thereof assumed and adopted the name Laureen Awino Otwala, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Laureen Awino Otwala only.
