Box 244, Ahero in the Republic of Kenya, formerly known as Percy Awuor Asembo, formally and absolutely renounced and abandoned the use of her former names Percy Awuor Asembo and in lieu thereof assumed and adopted the name Percy Awuor Omollo, for all purposes and authorize and request all persons at all times to designate, describe and address her by her assumed name Percy Awuor Omollo only.
