Box 244, Ahero in the Republic of Kenya, formerly known as Patricia Awiti Otieno, formally and absolutely renounced and abandoned the use of her former names Patricia Awiti Otieno and in lieu thereof assumed and adopted the name Patricia Awiti Omollo, for all purposes and authorize and request all persons at all times to designate, describe and address her by her assumed name Patricia Awiti Omollo only.
