Box 47658, Nairobi in the Republic of Kenya, formerly known as Anne Dorothy Wairimu Weke, formally and absolutely renounced and abandoned the use of my former name Anne Dorothy Wairimu Weke, and in lieu thereof assumed and adopted the name Anne Dorothy Wairimu Nganga, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Anne Dorothy Wairimu Nganga only.
