NOTICE is given that by a deed poll dated 19th June, 2012, duly executed and registered in the Registry of Documents in Volume DI, Folio 127/2063, File No. MMXII, by our client, Madoka Ellis, of 31, Fifth Garden, Rustington, Little Hampton, West Sussex BN16 3 EW, United Kingdom, a resident of the United Kingdom, formerly known as Kiwinda Madoka Mwanyia, formally and absolutely renounced and abandoned the use of his former name Kiwinda Madoka Mwanyia, and in lieu thereof assumed and adopted the name Madoka Ellis, for all purposes and authorize and request all persons at all times to designate, describe and address him by his assumed name Madoka Ellis only.
