Box 33–00200, Nairobi, formerly known as Lenity Kagwiria Mwenda, formally and absolutely renounced and abandoned the use of her former name Lenity Kagwira Mwenda, and in lieu thereof assumed and adopted the name Lenity Kagwiria Mwenda Kibiti, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Lenity Kagwiria Mwenda Kibiti only.
