Box 39017–00623, Nairobi in the Republic of Kenya, formerly known as Aarti Vipul Premchand Haria, formally and absolutely renounced and abandoned the use of my former name Aarti Vipul Premchand Haria, and in lieu thereof assumed and adopted the name Manali Desai, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Manali Desai only.
