Box 287, Kitengela in the Republic of Kenya, formerly known as Emmanuel Simiyu Sikuku, formally and absolutely renounced and abandoned the use of my former names Emmanuel Simiyu Sikuku, and in lieu thereof assumed and adopted the name Emmanuel Simiyu Kimei Sikuku, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Emmanuel Simiyu Kimei Sikuku only.
