Box 289–30100, Eldoret in the Republic of Kenya, on behalf of Adrian Trevor Mua (minor), formerly known as Adrian Trevor Kigen, formally and absolutely renounced and abandoned the use of his former name Adrian Trevor Kigen and in lieu thereof assumed and adopted the name Adrian Trevor Mua, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Adrian Trevor Mua only.
