Box 75073–00200, Nairobi in the Republic of Kenya, formerly known as Dinah Achieng Oketch, formally and absolutely renounced and abandoned the use of her former name Dinah Achieng Oketch, and in lieu thereof assumed and adopted the name Dinah Selah Achieng Oketch, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Dinah Selah Achieng Oketch only.
