Box 24– 00232, Ruiru in the Republic of Kenya, on behalf of Jayden Mbugua (minor), formerly known as Joe Jayden Kimandu, formally and absolutely renounced and abandoned the use of his former name Joe Jayden Kimandu and in lieu thereof assumed and adopted the name Jayden Mbugua, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Jayden Mbugua only.
