Box 496–00618, Ruaraka in the Republic of Kenya, on behalf of Esperanza Zawadi Jacob (minor), formerly known as Esperanza Zawadi, formally and absolutely renounced and abandoned the use of her former name Esperanza Zawadi, and in lieu thereof assumed and adopted the name Esperanza Zawadi Jacob, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Esperanza Zawadi Jacob only.
