Box 228–80109, Mtwapa in the Republic of Kenya, on behalf of Sherlyn Nyawira Hamann (minor), formerly known as Sherlyn Nyawira, formally and absolutely renounced and abandoned the use of her former name Sherlyn Nyawira, and in lieu thereof assumed and adopted the name Sherlyn Nyawira Hamann, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Sherlyn Nyawira Hamann only.
