Box 1095–00518, Nairobi in the Republic of Kenya, on behalf Shanice Clare Odongo (minor), formerly known as Shanice Clare Achieng Odongo, formally and absolutely renounced and abandoned the use of her former name Shanice Clare Achieng Odongo, and in lieu thereof assumed and adopted the name Shanice Clare Odongo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Shanice Clare Odongo only.
