Box 823-80108, Kilifi in the Republic of Kenya, formerly known as Sophia Nthenya Odongo, formally and absolutely renounced and abandoned the use of my former names Sophia Nthenya Odongo and in lieu thereof assumed and adopted the name Sophia Nthenya Okeyo, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Sophia Nthenya Okeyo only.
