Box 75–30103, Kipkabus in the Republic of Kenya, formerly known as Anne Jeptoo Sawe, formally and absolutely renounced and abandoned the use of her former names Anne Jeptoo Sawe, and in lieu thereof assumed and adopted the name Priscah Cheptoo Sawe, for all purposes and authorize and request all persons at all times to designate, describe and address her by her assumed name Priscah Cheptoo Sawe only.
