Box 30767-00100, Nairobi in the Republic of Kenya, formerly known as Cynthiah Waithera Ndegwa, formally and absolutely renounced and abandoned the use of her former name Cynthiah Waithera Ndegwa riithi and in lieu thereof assumed and adopted the name Cynthiah Asha Amani, for all purposes and authorizes and requests all persons at all times, to designate, describe and address her by her assumed name Cynthiah Asha Amani only. * Dated the 16th June, 2016.
