Box 82251-80100, Mombasa, formerly known as Safaa Ahmed All, formally and absolutely renounced and abandoned the use of her former name Safaa Ahmed Ali, and in lieu thereof assumed and adopted the name Safaa Ahmed Ali Salim Dakik, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Safaa Ahmed Ali Salim Dakik only.
