Box 40502-00100, Nairobi in the Republic of Kenya, formerly known as Luvuno Patrick Kwesha Dzombo alias Rhoda Patrick Dzombo alias Rhoda Wabukala, formally and absolutely renounced and abandoned the use of her former name Luvuno Patrick Kwesha Dzombo alias Rhoda Patrick Dzombo alias Rhoda Wabukala and in lieu thereof assumed and adopted the name Rhoda Luvuno Wabukala, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Rhoda Luvuno Wabukala only.
