Box 34540– 00100, Nairobi in the Republic of Kenya, formerly known as Juliet Janet Wanjiku alias Janet Becky Wanjiku alias Wanjiku Janette Becky alias Wanjiku Janet Becky alias Janet Becky Wanjiku alias Inga’a Janet Wanjiku B formally and absolutely renounced and abandoned the use of her former names Juliet Janet Wanjiku alias Janet Becky Wanjiku alias Wanjiku Janette Becky alias Wanjiku Janet Becky alias Janet Becky Wanjiku alias Inga’a Janet Wanjiku B, and in lieu thereof assumed and adopted the name Janet Wanjiku Ingaa for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Janet Wanjiku Ingaa only.
