Box 4565-00200, Nairobi, formerly known as Isaac Kaman Guchayo, formally and absolutely renounced and abandoned the use of his former name Isaac Kamau Guchayo and in lieu thereof assumed and adopted the name Isaac Meja Kamau Guchayo, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Isaac Meja Kamau Guchayo only.
