Box 40111- 00100, Nairobi in the ReRublic of Kenya, formerly known as Rachel Gathoni Muema Mbai, formally and absolutely renounced and abandoned the use of her for name Rachel Gathoni Muema Mbai and in lieu thereof assumed and adopted the name Rachel Gathoni Mbai, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Rachel Gathoni Mbai only.
