Box 43342- 80100, Nairobi in the Republic of Kenya, formerly known as Erica Connie Wakisha Nyange, formally and absolutely renounced and abandoned the use of my former name Erica Connie Wakisha Nyange and in lieu thereof assumed and adopted the name Erica Constance Wakisha, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Erica Constance Wakisha only.
