Box 86215-80100, Mombasa in the Republic of Kenya, formerly known as Sharad Dhirajlal Bhatti, formally and absolutely renounced and abandoned the use of my former name Sharad Dhirajlal Bhatti, and in lieu thereof assumed and adopted the name Sharad Dhirajlal Shamji Bhatti for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Sharad Dhirajlal Shamji Bhatti only.
