Box 110–80100, Mombasa in the Republic of Kenya, on behalf of Amar Neymar Chiaro Sarkkinen (minor), formerly known as Amar Miroshnikov formally and absolutely renounced and abandoned the use of his former name Amar Miroshnikov, and in lieu thereof assumed and adopted the name Amar Neymar Chiaro Sarkkinen, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Amar Neymar Chiaro Sarkkinen only.
