Box 8376–00300, Nairobi in Kenya, on behalf of Bolt James Maina (Minor), formerly known as Bolt Ivuti Ndiso, formally and absolutely renounced and abandoned the use of his former name Bolt Ivuti Ndiso, and in lieu thereof assumed and adopted the name Bolt James Maina, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Bolt James Maina only.
