Box 77588-00611, Nairobi in the Republic of Kenya, on behalf of Azariah Mumbua (minor), formerly known as Azariah Lydiah Mumbua, formally and absolutely renounced and abandoned the use of her former name Azariah Lydiah Mumbua, and in lieu thereof assumed and adopted the name Azariah Mumbua, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Azariah Mumbua only.
