Box 65903–00607, Kamiti in the Republic of Kenya, formerly known as Faith Cherono Baluzis, formally and absolutely renounced and abandoned the use of her former name Faith Cherono Baluzis, and in lieu thereof assumed and adopted the name Faith Cherono Baluziene, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Faith Cherono Baluziene only.
