Box 19323–00202, Nairobi in the Republic of Kenya, formerly known as Masitsa Naomi Shiyonga Mama Signal, formerly and absolutely renounced and abandoned the use of her former name Masitsa Naomi Shiyonga Mama Signal, and in lieu thereof assumed and adopted the name Masitsa Naomi Shiyonga, for all purposes and authorizes and requests all persons to designate, describe and address her by her assumed name Masitsa Naomi Shiyonga only.
