Box 6808-00100, Nairobi in the Republic of Kenya, formerly known as Shelmith Wanjugu Kennedy Mutie, formally and absolutely renounced and abandoned the use of her former name as Shelmith Wanjugu Kennedy Mutie and in lieu thereof assumed and adopted the name Shelmith Wanjugu Jemno Macharia, for all purposes and authorizes and requests all persons at all times to designate describe and address her by her assumed name Shelmith Wanjugu Jerono Macharia only.
