Box 41665–00100, Nairobi in the Republic of Kenya, formerly known as Yvonne Rahab Wanjiku Gitau, formally and absolutely renounced and abandoned the use of her former name Yvonne Rahab Wanjiku Gitau, and in lieu thereof assumed and adopted the name Yvonne Rahab Wanjiku Kimondo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Yvonne Rahab Wanjiku Kimondo only.
