Box 60497–00200, Nairobi in the Republic of Kenya, on behalf of Hanan Nahla Zoe Juma (minor), formerly known as Hannan Ifrah Munuhe, formally and absolutely renounced and abandoned the use of her former name Hannan Ifrah Munuhe and in lieu thereof assumed and adopted the name Hanan Nahla Zoe Juma, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Hanan Nahla Zoe Juma only.
