Box 630, Kiserian in the Republic of Kenya, on behalf of Arya Sara Muthoni Ndegwa (minor), formerly known as Arya Sara Muthoni Elenga, formally and absolutely renounced and abandoned the use of her former name Arya Sara Muthoni Elenga, and in lieu thereof assumed and adopted the name Arya Sara Muthoni Ndegwa, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Arya Sara Muthoni Ndegwa only.
