Box 1822– 30100, Kitale in the Republic of Kenya, formerly known as Valentine Nelima Wanambisi, formally and absolutely renounced and abandoned the use of her former name Valentine Nelima Wanambisi, and in lieu thereof assumed and adopted the name Valentine Neli Wanambisi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Valentine Neli Wanambisi only.
