Box 82357–80100, Mombasa in Kenya, formerly known as Joshua Murimi Ndiga, formally and absolutely renounced and abandoned the use of his former name Joshua Murimi Ndiga, and in lieu thereof assumed and adopted the name Joshua Ndiga Kubutha, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Joshua Ndiga Kubutha only.
