Box 1482–00217, Limuru in Kenya, formerly known as Julia Shalom Wangui Gichuhi, formally and absolutely renounced and abandoned the use of my former name Julia Shalom Wangui Gichuhi, and in lieu thereof assumed and adopted the name Shalom Wangui Gichuhi, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Shalom Wangui Gichuhi only.
