Box 80366–80100, Mombasa in Kenya, formerly known as Jackline Kagendo M’Inoti, formally and absolutely renounced and abandoned the use of her former name Jackline Kagendo M’Inoti and in lieu thereof assumed and adopted the name Jackline Kagendo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jackline Kagendo only.
