Box 48–3050, Garissa in the Republic of Kenya, formerly known as Mohamud Mohamed Afey, formally and absolutely renounced and abandoned the use of my former name Mohamud Mohamed Afey, and in lieu thereof assumed and adopted the name Ahmed Aden Abdullahi, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Ahmed Aden Abdullahi only.
