Box 76164–00508, Nairobi in the Republic of Kenya, on behalf of Drake Kolwa Flannigan (minor), formerly known as Drake Kolwa, formally and absolutely renounced and abandoned the use of his former name Drake Kolwa, and in lieu thereof assumed and adopted the name Drake Kolwa Flannigan for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Drake Kolwa Flannigan only.
