Box 76164– 00508, Nairobi in the Republic of Kenya, formerly known as Bella Raju Kolwa, formally and absolutely renounced and abandoned the use of my former name Bella Raju Kolwa, and in lieu thereof assumed and adopted the name Bella Raju Flannigan for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Bella Raju Flannigan only.
