Box 24322–00100, Nairobi in the Republic of Kenya, formerly known as Usha Ratilal Popatlal Shah, formally and absolutely renounced and abandoned the use of her former name Usha Ratilal Popatlal Shah and in lieu thereof assumed and adopted the name Usha Chandravadan Dipchand Shah, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Usha Chandravadan Dipchand Shah only.
